HEALTHY FOOD HUB
Black Oaks Center’s Healthy Food Hub provides weekly customized RX Therapeutic Food Bags for Pembroke families to prevent and treat high blood pressure. Disinvestment in Pembroke means that grocery stores and markets are scarce, leaving the township to struggle as a food desert. Our program stands in the gap, giving our families a chance at healthier lives and a brighter future.

Prepare the food as instructed/advised in the Healthy Food Hub Starter Kit to get the most medicine from the food.

Track and share your family's blood pressure readings to chart your progress.

Share any blood test results to help us monitor your progress in the program.
